DeQvervains disease: Causes, Diagnosis and Treatment

Pain in thumb and dragging pain in wrist is one of the common complaints in software employees. Its an occupational injury due to repeated stretching of tendons of thumb. The phenomena are called DeQvervains disease / DeQvervains tenosynovitis.
Thumb has 2 major tendons called Extensor pollicis brevis and abductor pollicis longus. These two tendons pass from wrist to thumb through a tunnel. they form components of first extensor compartment. When there is inflammation in this tunnel or its contents then happens DeQvervains disease.

Causes

Causes – repetitive strain injury - computer mouse, mobile phone addicts, fishing, golfing, sewing. Most common cause if idiopathic.

Diagnosis

  • Finkelstein test
  • Ultrasonography of wrist

Treatment options

DeQvervains disease has wide modes of management- 
  • Rest with splint
  • Plaster application
  • Resistant cases need surgery to release extensor retinaculum.
